Criminal History
Except for the specific questions set forth below, a Colorado public institution of higher education may not use, obtain, inquire into or require disclosure of, an applicant's criminal history or disciplinary history at another school or educational institution in connection with any application for admission.

While the following inquiries are permissible, an applicant has a right to appeal an admission decision that is based on his or her answers to these questions. See the Mines' Policy webpage for more information.
  • Do you have any criminal charges pending against you?
  • Do you have any prior disciplinary history at another academic institution for stalking, sexual assault or domestic violence?
  • Do you have any prior convictions* for stalking, sexual assault or domestic violence?
  • Do you have any prior convictions*, within five years before submitting the application, for assault, kidnapping, voluntary manslaughter or murder?
Applicants have an obligation to report any updates or changes to their criminal and disciplinary response if they are arrested and charged with any crime after completing their application for admission. Applicants are required to disclose any and all such information in writing to the Executive Director of Admissions at Mines or, once enrolled, to the Dean of Students. Failure to report changes that should have been disclosed may result in an allegation of a violation of the Mines Code of Conduct.
*Conviction means a conviction by a jury verdict or by entry of a verdict or acceptance of a guilty plea or a plea of nolo contendere by a court. Conviction does not include a plea to a deferred judgment and sentence until the deferred judgment and sentence are revoked. Criminal convictions that have been expunged, sealed, annulled, pardoned or otherwise required by law or ordered by a court to be kept confidential need NOT be disclosed.
Identity Verification
Upon submission of this form, you will be asked to complete an identity verification process. This will only take a few minutes and requires that you have access to a smartphone and your identity documents, such as a driver's license or passport.

Please verify your identity as soon as possible; your application cannot be processed until complete. Verifying your identity helps us maintain your records and ensure your account is accurate and secure. Identity information is not shared and is only used for the purpose of your application account.
Certify Accuracy
I certify that the information on this application is complete and correct, and I understand that the submission of false information is grounds for denial of my application, withdrawal of any offer of acceptance, cancellation of enrollment, or any appropriate corrective action. I authorize Colorado School of Mines to verify the information I have provided with all of the school(s) that I have attended. I agree to notify the proper officials of the institution of any changes in the information provided.
